Cave Martial Arts Academy – Participant Waiver & Consent

By completing this booking, I confirm that I am either the participant or the parent or legal guardian of the participating child and agree to the following:

1. Assumption of Risk

I understand that martial arts involves physical activity and carries a risk of injury. I accept full responsibility for mine or my child’s participation and acknowledge that Cave Martial Arts Academy will take reasonable steps to provide a safe training environment.

2. Medical Fitness

I confirm that myself/my child is fit to take part in physical activity. I agree to inform the academy of any medical conditions, injuries, allergies, or additional needs that may affect participation. I consent to basic first aid being administered if required.

3. Limitation of Liability

I understand that Cave Martial Arts Academy, its instructors, and staff are not liable for injuries that may occur during training, except where caused by negligence. I agree that my child participates at their own risk.

4. Behaviour & Conduct

I understand that respectful behaviour is required at all times. The academy reserves the right to remove any participant whose behaviour poses a risk to themselves or others.

5. Photography & Media (Optional)

I give permission for photos or videos of myself/my child taken during classes to be used for academy marketing and social media.

6. Data Protection

I consent to my information being stored and used for class administration in accordance with UK data protection laws. My details will not be shared with third parties.

7. Agreement
